What are the types of hypertension?

Hypertension, commonly known as high blood pressure, is classified into two main types: primary (essential) hypertension and secondary hypertension. These classifications help differentiate between cases where hypertension is the primary health issue and cases where it's a result of an underlying medical condition.

Primary (Essential) Hypertension:

Primary hypertension is the most common type and accounts for the majority of hypertension cases. It develops gradually over time and tends to have no identifiable cause. Several factors, such as genetics, age, lifestyle choices, and environmental factors, can contribute to primary hypertension. While the exact mechanisms are not fully understood, these factors can lead to increased resistance in the arteries, resulting in elevated blood pressure.

Secondary Hypertension:

Secondary hypertension occurs as a result of an underlying health condition or specific factors that directly cause elevated blood pressure. Identifying and treating the underlying cause of secondary hypertension can sometimes lead to blood pressure returning to normal. Various medical conditions and factors can contribute to secondary hypertension, including:

  • Kidney problems: Conditions such as chronic kidney disease, renal artery stenosis, and polycystic kidney disease can lead to increased blood pressure.
  • Hormonal disorders: Disorders like primary aldosteronism, Cushing's syndrome, and thyroid disorders can contribute to hypertension.
  • Medications: Certain medications, such as nonsteroidal anti-inflammatory drugs (NSAIDs), corticosteroids, and some decongestants, can cause elevated blood pressure.
  • Obstructive sleep apnea: This sleep disorder is linked to hypertension due to its impact on breathing and oxygen levels during sleep.
  • Endocrine disorders: Conditions affecting the endocrine system, like pheochromocytoma and hyperparathyroidism, can lead to secondary hypertension.
  • Coarctation of the aorta: A narrowing of the aorta can cause high blood pressure in the arms and head while being lower in the legs.
  • Pregnancy: Some women develop high blood pressure during pregnancy (gestational hypertension) or have a preexisting condition exacerbated by pregnancy (preeclampsia).

In addition to primary and secondary hypertension, there are a few other specific types of hypertension that are worth mentioning:

Isolated Systolic Hypertension (ISH):

This type of hypertension is characterized by elevated systolic blood pressure (the top number in a blood pressure reading) while the diastolic blood pressure (the bottom number) remains within normal range. ISH is more common in older adults and is often related to the stiffening of arteries that occurs with age.

Malignant Hypertension:

Also known as hypertensive crisis, malignant hypertension is a severe form of high blood pressure that can lead to organ damage. It requires immediate medical attention. Blood pressure readings in this condition are extremely high and can cause symptoms like severe headache, blurry vision, chest pain, and shortness of breath.

White Coat Hypertension:

This refers to a situation in which a person's blood pressure is elevated only when measured in a medical setting, such as a doctor's office, but remains within normal range when measured at home. This phenomenon can be attributed to anxiety or stress related to medical visits.

Masked Hypertension:

In contrast to white coat hypertension, masked hypertension occurs when a person's blood pressure is normal in a medical setting but elevated when measured at other times, such as at home. It's important to identify masked hypertension because it carries similar cardiovascular risks as sustained hypertension.

Resistant Hypertension:

This type of hypertension refers to cases where blood pressure remains high despite treatment with multiple antihypertensive medications. Managing resistant hypertension can be challenging and often requires close monitoring and adjustments to the treatment plan.

Renovascular Hypertension:

This type of hypertension is caused by narrowed or blocked arteries leading to the kidneys (renal arteries). It's often associated with conditions like renal artery stenosis and can lead to secondary hypertension due to the kidneys' role in regulating blood pressure.

Sustained Hypertension:

Sustained hypertension refers to consistently elevated blood pressure readings over an extended period of time. It's the most common presentation of hypertension and can include both primary and secondary forms. Sustained hypertension is typically diagnosed when multiple blood pressure measurements taken on different occasions consistently show elevated readings.
